While isolated primary biceps tendinitis accounts for less than 5% of presentations, secondary biceps tenosynovitis is extraordinarily prevalent, almost invariably coexisting with rotator cuff tears, subacromial impingement, or labral pathology. The long head of the biceps plays a secondary role as a dynamic anterior stabilizer and humeral head depressor. Chronic friction within the groove, combined with subluxation of the transverse humeral ligament, results in disabling anterior shoulder pain that responds exceptionally well to targeted, non-operative physiotherapy.

Pathophysiology & Biomechanical Impact

The long head of the biceps enters the shoulder joint through the bicipital groove, held securely by the transverse humeral ligament and the sling fibers of the coracohumeral ligament and subscapularis. Because the synovial sheath of the tendon communicates directly with the glenohumeral joint cavity, any intra-articular inflammation rapidly translates into tenosynovitis. Repetitive gliding under tensile and compressive load causes sheath thickening, vascular proliferation, and fibrillar delamination. If the stabilizing sling tears, the tendon repeatedly subluxates over the lesser tuberosity, causing intense mechanical irritation and eventual structural failure.
