Tarsal Tunnel Syndrome (TTS) Management

Tarsal Tunnel Syndrome is a focal entrapment neuropathy caused by mechanical compression of the posterior tibial nerve or its branches (medial and lateral plantar nerves) beneath the flexor retinaculum on the medial aspect of the ankle. Targeted physical therapy decompresses the fibro-osseous canal, corrects hindfoot biomechanics, and mobilizes neural tissues.

Clinical Presentation

Symptoms & Functional Limitations

Patients suffering from tarsal tunnel syndrome (tts) management frequently experience the following localized and kinetic symptoms:

1
Burning pain, tingling (paresthesia), and numbness radiating along the plantar sole and toes
2
Sharp pain aggravated by prolonged standing, walking, or running
3
Positive Tinel sign with percussion over the retromalleolar medial canal
4
Cramping sensation in the intrinsic foot muscles and arch fatigue
5
Worsening discomfort at night or after prolonged dependent foot positioning
Clinical Red Flag Notice:If you experience sudden loss of bowel/bladder sensation, high fever with joint swelling, or sudden total inability to bear weight, seek immediate hospital emergency care before beginning physical therapy.
Etiology & Pathomechanics

Root Anatomical & Structural Causes

Understanding the underlying mechanical factors helps us formulate a personalized protocol targeted at long-term recovery:

01Etiological Factor
Excessive subtalar pronation and pes planus (flat feet) causing neural stretch traction
02Etiological Factor
Post-traumatic fibrosis and scar tissue formation following ankle sprains or malleolar fractures
03Etiological Factor
Space-occupying lesions such as ganglion cysts, varicose veins, or flexor tendon tenosynovitis
04Etiological Factor
Systemic fluid retention (pregnancy, hypothyroidism, renal insufficiency)

Evidence-Based 3-Phase Rehabilitation Protocol

Our physiotherapists follow a phased clinical blueprint approved by senior consultants, progressing you safely from pain relief to full independence:

1

Phase 1: Neural Decompression & Anti-Inflammatory Care

Step 1 of 3

Reducing canal pressure through gentle unloaded positioning, anti-inflammatory modalities, and temporary biomechanical medial arch unloading.

Targeted Bedside Modalities & Exercises Applied:
Targeted Ultrasound Therapy Cryo-Compression Custom Arch Taping Gentle Unloaded Active Ankle Pumps
2

Phase 2: Tibial Nerve Mobilization & Soft Tissue Release

Step 2 of 3

Graded neural tension gliding (dorsiflexion-eversion flossing), deep flexor retinaculum myofascial release, and gastroc-soleus flexibility restoration.

Targeted Bedside Modalities & Exercises Applied:
Tibial Nerve Gliding Drills Myofascial Release of Deep Calf Stretching of the Flexor Hallucis Longus IFT for Neural Desensitization
3

Phase 3: Biomechanical Kinetic Correction & Strengthening

Step 3 of 3

Strengthening the posterior tibialis and intrinsic foot musculature to maintain the medial longitudinal arch and prevent chronic nerve tethering.

Targeted Bedside Modalities & Exercises Applied:
Short-Foot Intrinsic Muscle Activation Towel Curls & Marble Pickups Eccentric Tibialis Posterior Loading Custom Orthotic Advice

Specialized Modalities Brought to Your Bedside

You do not need to buy expensive machines or travel to clinics. Every Aries PhysioCare therapist arrives fully equipped with certified electrotherapy and rehabilitation tools:

Portable IFT & TENS

Deep electrical current frequencies that intercept pain signals and accelerate tissue oxygenation.

Therapeutic Ultrasound

High-frequency sound waves that soften scar tissue, relax deep spastic muscles, and reduce swelling.

Mobilization Belts

Clinical mobilization belts for precision joint gapping and decompression under safe manual guidance.

Cryo-Compression

Targeted pneumatic compression cuffs and ice therapy to rapidly eliminate post-traumatic fluid buildup.

Kinesiology Taping

Neuromuscular tape applications to stabilize unstable joints, decompress fascia, and guide lymphatic drainage.

Theraband & Resistance Kit

Graduated resistance bands, balance foam pads, and hand dynamometers for measurable strength gains.

Patient-Centric Advantages

Why In-Home Physiotherapy for Tarsal Tunnel Syndrome (TTS) Management?

Recovering in your natural living environment provides distinct clinical advantages over crowded outpatient centers:

Patients experiencing severe plantar neuropathic pain aggravated by prolonged transit and clinic waiting
Individuals needing personalized gait evaluation on their real living flooring surfaces (tile, wood, carpet)
Seniors at risk of fall due to loss of plantar protective sensation
